Online Course Evaluation Report Access

All faculty have inherent access to their own course evaluations through DuckWeb. Simply log on to DuckWeb with your UO ID and Personal Access Code (PAC), then click on the Course Evaluations menu item. Department Chairs and Deans automatically have access to course evaluation data for faculty in their units as permitted under Faculty Legislation. Other faculty and administrative staff who need access to course evaluation reports must complete the access request form available via the link below.

Each person must sign a Code of Responsibility statement when requesting course evaluation report access.

Steps for Obtaining Course Evaluation Report Access

1. Download, complete, and return the Access Request Form (PDF)
2. When access has been granted, the Registrar’s Office will notify you by e-mail that your access is open.
3. Access DuckWeb at https://duckweb.uoregon.edu and click on Course Evaluations to get started.
