Name Changes
Students may change their names on their official academic record by submitting a Name Change Request (including a document from the list below) to the
Registrar's Office. Name Change Requests that are missing the corresponding documentation will not be processed. Current and former employees should contact the
Payroll Office to have their names changed, even if the employee has a student record.
Name Change Requests must include one of the following documents (photocopies permitted):
- A valid driver’s license
- A valid state ID
- A valid passport
- A birth certificate
- A marriage license
- A divorce decree authorizing the name change
- A judicial decree specifically authorizing a name change
- Naturalization papers
Students who have already
applied to graduate and wish to have the new name appear on their diploma must check the corresponding box provided on the form.
International Students
If you are not a US citizen, the name on your UO records must match the name on your passport. A copy of this document must be attached for processing. If you have questions about this policy, contact the

Address Changes
It is essential that students maintain a current mailing address on file with University of Oregon. This information is accessed by all offices to send critical items such as bills, scholarship information, diplomas, and more. To change your address, log into DuckWeb and follow the steps below; this action updates your record immediately.
Students who attended the University of Oregon since fall 1986 have access to change their address via
DuckWeb. Need a PAC reset? Contact us at the information below.
How to Change Your Address Through Duckweb
- Login to DuckWeb using your UO ID Number and PAC
- Select "Personal Information"
- Select "Updated Address(es) and Phone(s)"
- Click on the link next to the address or phone number you wish to update
- After making changes, click Submit and review for accuracy
If you do not have a
DuckWeb account, you may mail, fax, or email (from your uoregon.edu email account only) your address change along with your UO ID number and signature to:
