Name, Gender, Address, and Ethnicity Changes

Name and/or Gender Changes

Students may change their name and/or gender marker on their official academic record by submitting a Name and/or Gender Change Request to the Registrar's Office. Current and former employees should contact the Payroll Office to have their name changed, even if the employee has a student record.

Students who have a pending application to graduate who wish to have the new name appear on their diploma must check the corresponding box provided on the form.

Name Change Requests to the Office of the Registrar require one of the following documents (photocopies permitted):

  • A valid driver’s license
  • A valid state ID
  • A valid passport
  • A birth certificate
  • A marriage license specifically stating the new name after marriage
  • A divorce decree authorizing the name change
  • A judicial decree specifically authorizing a name change
  • Naturalization papers stating the change of name

Students who do not have a valid copy of one of these documents should submit the form to LGBT Education and Support Services.

International Students

If you are not a US citizen or Permanent Resident Card holder, for visa reasons, the legal name on your UO records must match the name on your passport. A copy of your passport must be attached for processing. If you have questions about this policy, contact the International Affairs Office at (541) 346-3206 or visit them in person at 300W Oregon Hall. If you wish to use a different first name within UO, you may use the preferred first name function described below.

How to Enter or Update your Preferred First Name Through DuckWeb

  1. Login to duckweb.uoregon.edu using your UO ID Number and PAC
  2. Select "Personal Information"
  3. Select "Enter/Update Preferred First Name"
  4. Enter your preferred first name in the text box provided, then click "Submit"

If you do not have your DuckWeb login information, you may use the Name/Gender Change Request Form to submit your preferred first name. If this is all that you are updating, no documentation is required.

Preferred first name appears on class lists, in Canvas, on most advising and student pages in DuckWeb, and in the Find People online directory. Note that Find People is refreshed overnight. If you want both your Legal and Preferred First Name to appear in Find People, enter both of your names as your preferred name, as follows: William "Bill" (with the quotation marks).

Address Changes

It is essential that students maintain a current mailing address on file with University of Oregon. This information is accessed by all offices to send critical items such as bills, scholarship information, diplomas, and more. To change your address, log into DuckWeb and follow the steps below; this action updates your record immediately.

Students who attended the University of Oregon since fall 1986 have access to change their address via DuckWeb. Need a PAC reset? Contact us at the information below.

How to Change Your Address Through DuckWeb

  1. Login to duckweb.uoregon.edu using your UO ID Number and PAC
  2. Select "Personal Information"
  3. Select "Updated Address(es) and Phone(s)"
  4. Click on the link next to the address or phone number you wish to update
  5. After making changes, click Submit and review for accuracy

If you do not have a DuckWeb account, you may mail, fax, or email (from your uoregon.edu email account only) your address change along with your UO ID number and signature to:

Ethnicity Add or Change Requests

Students who may not have reported their ethnicity when entering the University of Oregon may do so using the Ethnicity Add/Change Form. Alternatively, students may have reported their ethnicity previously and may now choose to supply more complete and accurate information about their ethnicity.

Students completing the Ethnicity Add/Change form should mailed, faxed, or delivered in person to:
