Name Description
Consent Form for Releasing Education Records For students requesting the release of non-directory information. Other written formats are acceptable if they include:
  1. What information is to be released
  2. To whom the information is to be released
  3. The purpose for which it is to be released
  4. Your signature and the date signed
Consent Form for Courses Using Blogs For faculty using blogs in courses, collect this form from each of your students.
Consent Form for Requesting References/Recommendations For students requesting letters of reference or recommendation from University of Oregon faculty or staff members.
Code of Responsibility for Education Records For departments to use when students/staff without access to systems are handling records.
Random Number Generator Faculty and staff can use the Random Number Generator to generate code numbers for students enrolled in their classes. (Graded papers, exams, quizzes and other assignments are considered part of the student's education record, and must be returned to the student in a manner designed to maintain confidentiality.)
Consent Form for ePortfolio Participation For faculty using ePortfolio, collect and retain this form from each of your students.
Banner form SWIFERP A Banner form available to all Faculty and Staff with Banner access. If the student does not have a directory restriction, all information on this form is releasable to a third party with the exception of the student ID number.
Consent Forms for Photos and Videos Use when taking photos and videos of students for use on the web and in print. Use either waiver when producing content involving students.
