The Office of the Registrar can assist students with the notarization of official transcripts, the original or a copy of their UO diploma, or other documents related to the UO student record. A $10 service fee is required for each notarized document.
Official Transcript
There is a $15 fee for each official transcript, in addition to the $10 service fee required for each notarized document. Please use the button above to request notarization.
Other Documents
Please use the button above to request notarization of a degree certification letter, enrollment verification, online course letter, or a copy of the UO diploma. Notarized documents will not be emailed or faxed. Please provide the mailing address if you would like the document(s) mailed. Indicate "pick up" if you prefer to pick up the documents at the Office of the Registrar.
Replacement Diploma
If you need to order a replacement diploma and need that replacement to be notarized, please place your order through the Online Diploma Service and add the option for Notary Service.
You cannot use the Request for Notarization form to order a notarized replacement diploma.
