Name, Gender, and Ethnicity Changes

Students and alumni can make updates to their name, gender identity, and ethnicity in their student record.

Name Changes

The Office of the Registrar updates names on a student's record upon request. Students must submit a Name and/or Gender Change Request form to initiate the process.

Name and/or Gender Change Request

A copy of one of the following documents is required to update a student's name:

  • A valid driver’s license
  • A valid state ID
  • A valid passport
  • A birth certificate
  • A marriage license stating the new name after marriage
  • A divorce decree authorizing the name change
  • A judicial decree authorizing a name change
  • Naturalization papers stating the name change

Students who do not have a valid copy of one of these documents should submit the form to LGBT Education and Support Services (LGBTESS).

International Students

Students who are attending the University of Oregon with an F-1 and J-1 visa must have their legal name on university records match the name of their passport and immigration documents provided by the Division of Global Engagement (DGE). Questions about the policy may be addressed to the advisors in DGE by calling (541) 346-3206 or visiting them at 300W Oregon Hall. International students who are interested in changing their name at the UO but do not have the documentation may update their preferred name instead.

Preferred Name

You may change your preferred name in DuckWeb. If you do not have your DuckWeb access, you may use the Name/Gender Change Request Form to submit your preferred first name.

Preferred names appear on class lists, in Canvas, on most advising and student pages in DuckWeb, and in the Find People online directory.

The Preferred First Name policy can be found in the UO Policy Library.

Updates to Gender

A student may update their gender marker in their student record by completing the Name and/or Gender Change Form. No documentation is needed to take this action. Students who would like support when making this update may visit LGBTESS.

Updates to Reported Ethnicity

Students who wish to update their ethnicity information may do so in DuckWeb. This allows students to accurately report their ethnicity information as they continue their studies at the university.
