ePortfolio Participation
If you require students to create an electronic portfolio for your course, students must complete a consent form.
Public-Facing Media Consent
If your classroom requires students to create a blog, social media account, or other public-facing media, students should complete the form which confirms their understanding and consent to complete the assignments.
Photo Waivers
If you would like to include student photos in web and print, you must have signed photo waivers. Either photo waiver is acceptable.
